Safe isolation first, then the job.

On a chemical site the job does not start when the technician arrives. It starts when the energy is off, the locks are on, and a second person has proved there is nothing left in the line.

Permit to Work holds that as a record. One job, one place, one time window, named people. The issue gate is real: a missing precaution, an unverified isolation, a missing gas test or a competency gap blocks the issue at the desk, before the crew is standing at the machine. LOTO (lock out, tag out) is two people, one to apply and one to verify, and every lock applied right now sits on one board a supervisor can open before start-up. A permit cannot be closed while a lock is still on.

Underneath it, HSE (health, safety and environment) owns the incident register, the observations and the hazard list the permits draw on. Maintenance runs the asset register, the work orders and the preventive maintenance rules, and starting a job can ask Permit to Work first. Utilities & Energy meters what the site consumes per carrier and per line, and puts the unaccounted share on a dashboard, which is usually where a leak is hiding. Calibration proves the instruments the process relies on.

The rules you work under

Chemical sites work under Seveso III for major-accident hazards and under REACH and CLP for substances and labeling, and most hold ISO 45001 and ISO 14001.
