HSE stands for Health, Safety and Environment. The module works in both directions. Looking back, one register holds injuries, near misses, property damage and environmental releases, and each one carries its investigation. Looking forward, it holds observations, inspections, and the hazard and risk register. One shared action list sits under all of it.
A supervisor reports the incident and names the people involved. The injury is classified on each person, not on the event. The investigator works through five why steps and writes the root cause. Every action gets an owner, a date and a mandatory flag. The case will not close until each mandatory action is closed, and the investigator cannot sign off their own investigation. Rates need hours worked, so the plant either types the payroll figure or derives it from the published shift plan. When the hours are missing the rate shows blank, not zero.
HSE owns the hazard list. Permit to Work reads it, so the hazards behind a permit type come from the same register the safety manager maintains.

What you get
- Any user can record an observation, including anonymously, so near misses get written down instead of shrugged off.
- Risk is scored on a 5x5 grid: how likely against how bad. Controls follow the standard order of eliminate, substitute, engineer, administer, protect.
- The two rates a board asks for, lost time injury frequency and total recordable incidents, are computed from hours the plant supplies. The module will not invent hours.
- A serious incident closes only when a second person signs it off and no mandatory action is left open.
- Every safety action carries an owner and a date, and the ones that become machine work go to Maintenance as a job.
- It supports ISO 45001 for safety and ISO 14001 for environment, the certificates many plants already hold.
