Permit to Work

Nobody works on a machine until it is safe and locked out.

Nobody works on a dangerous machine until it is safe. A permit is a written authority for one job, in one place, for one time window, with named people on it. Behind the permit sits LOTO, lock out and tag out: somebody switches the energy off, locks the switch and hangs a tag, and a second person proves there is no energy left.

The requester raises the permit and names the work party. Precautions copy in from the permit type. The work party is checked against the live skill matrix and the result is frozen on the permit. A missing precaution, an unverified isolation, a missing gas test or a competency gap blocks the issue, and the screen names the reason. That gap shows at the desk rather than at the machine with the crew already standing there. The permit will not close while a lock is still applied.

Competency comes from Skills & Training, not from a tick box on the permit. Starting a maintenance job can ask this module first, so the job and the permit stay in step.
